Are you a Dentist or a patient? GBR is technique and material sensitive. you have a 3-4 wall defect. it is extremely difficult to gain bone vertically. those molars are hopeless. I am assuming you will extract #31, 32 and try to gain bone back by GBR. This case is hopeless because, even if you able to grow bone back to the alveolar crest which is the best result anyone can achieve. you will not have enough vertical dimension for implant placement. I havn’t seen the rest of mandible. but most likely your patient will have to be content with RPD or remove rest of teeth for a lower overdenture w/ 2-3 implants or implant supported prosthesis (AOX). the chance of you gaining bone back with GBR at #31,32 is very low. good luck.

Thanks for your response. Patient is young and unmarried and hence am trying to give it a best shot.

honestly those teeth looks very old and beat up. Not much of a choice other than extraction. And what you planning for that site after you remove the molars?

Will build bone with grafts and membranes. Hopefully should get sufficient bone for implants after 4 months

Have you considered omitting the middle fixture and going with a 2x implant supported fixed bridge? The risk of the additional fixture, IMHO, outweighs any potential benefit.

Yes Dr., I have. Am planning to place the 1st implant (1st premolar region) while doing an open extraction for the first molar, and will do socket preservation for the extraction site and watch for a few months.

Thereafter, a 3 unit implant supported bridge. The cbct pic is only a simulation sent to me.
