Implant Size for mandibular molar region?

Hello Respected Doctors, I am planning to place 5× 11.5 implant at 36 region. Kindly look at the CBCT report and guide me if it is ok to place the above said diameter implant at 36 region! Also suggest treatment plan for 34 region! Thank you.


Safe distance between implant and canalis mandibula is 2mm, and if you placed 11,5mm implant you need to drill 12 - 12,5mm so implant placement is 0,5-1mm subcrestal to compensate future bone resoption

from cbct report you have 13,9mm, if i were you i prefer play safe and use 5x10,5mm implant

i rarely place any implant over 10mm in the posterior mandible. Definitely agree w/ Dr duvalie. As for 34 region. looks like there’s some buccal bone defect. 34 site will probably benefit from horizontal GBR before implant placement. But before your implant and GBR surgery, definitely address the LRQ. Any kind of active caries and perio might compromise your implant/GBR outcome.

I agree as I rarely place a fixture over 10mm long in the posterior unless it is an immediate and I am looking for some apical engagement. While I don’t recall the exact publication I do remember reading some papers 10-12 years ago looking into short vs. long implants and their documented success. The take home message from these now old studies was that an 8mm fixture had the same documented success as a longer fixture so I elected to stick with 8 and 10mm fixtures when placing in native posterior sites.

Good job dr.bikas, may i know why you shift from diameter 5mm to 4,5mm?

dr scotty would have placed a tissue level implant. someday i will try a tissue level implant in the posterior.

I understand your question as I was trained on more conventional fixture designs where fixture diameter corresponds to restorative platform. With these newer conical/universal platform designs does it make a difference what diameter fixture is used since everything emerges from the same "universal’ platform?? While I would never recommend placing a 3.5mm diameter for a molar it makes no difference in terms of emergence because everything is now the same. In other words this 4.5 is essentially the same as a 5.0 or even a 6.0. I am not convinced that oversimplifying restorative platforms is a good idea…
