Implant journey from 2016

i placed this implant (biohorizons) from 2016 and restored it 3 month later

The patient never back for routine follow up, and at june 2024 the patient back with the implant dislodged by it self for unknown reason, my guess is because bad oral hygine

After scaling and motivate the patient to enhance oral hygine, i perform bone graft to fill the socket and vertical height, this pic is 4 month after bone graft

After that i placed new implant with digital surgery guided and flapless technique

i think the implant should be placed a little more deeper, its a little different with the plan on cbct

im pretty sure already follow all the depths protocol on digital “plastic” guide, what do you think is the digital guide is not reliable? And will the implant survive in long term?

Looks good. I speak to a lot of people who strictly follow protocols provided by the plastic guide and they are often disappointed when the final outcome doesn’t match the digital plan. I have never used one of these guides though I have benefited from analog twist drill stent/guides made from a stone model. I have found that most folks who utilize these digital guides become overly dependant on them and fail to objectively analyze their work in real time while just accepting the end result because “the guide” told them to place it there. While I think the placement looks good in this case it might be helpful to periodically remove the plastic guide and do some free hand modifications to achieve your desired outcome. Nice work!!!

How is the soft tissue ?

Bone probably thin on the buccal.

Flapless and guide can create over heating of the bone.

Soft tissue is healthy and thick around 3-4mm

The guide hole is not full circle, its about ¾ circle so there is some space for irrigation, also i put the saline irrigation in refrigerator before surgery, i think the saline temperature about 5-10⁰C

The drill protocol from implant manufacture also a little different for example if you want to drill 10mm depth you need to use 6mm first, than 8mm and final 10mm, its not recomended to osteotomy with 10mm drill directly because it will generate more heat

Welcome to the world of implant dentistry, where on occasion implants will fail for no apparent reason. I’ve had that happen to me and if you do implants long enough , failures wiil occur. Don’t blame yourself.
