Dr. Helm totally correct. beautiful explanation. Conservative and passive splinting (using composite and wire. NOT tray) for 6-8 weeks is the key to success. Don't forget rx peridex. You are just prolonging the life of the tooth. Max central incisor is most common teeth to be injured for kids due to trauma. CT scan does not show any root fractures, which improves prognosis. Eventually pt will lose that tooth but hopefully with your help, it will be after patient is 30 years old. Since most of tooth structure is intact, shouldn't need a post. So just build up with composite should be fine. Dr. Helm basically summed up dental trauma chapters from pathway of the pulp and contemporary oral and maxillofacial surgery textbooks. I think he only left out how long to splint the teeth. i think most dentist place a post b/c we get paid for it. but there is a purpose to placing a post in tooth. strengthening the tooth is not the reason. not all endo teeth need a post placed. however, build up is 100% necessary. just like most of the full mouth implants i see presents on this forum here have no thought in the restorative side. they think about more implants = more money and did not consider the restorative aspects of full arch/mouth rehab. that's why these dentists run into problem after implants healed and scrambling restoring implants. by the way, it's been a month. how's the patient doing? any post op photos and radiographs?
